Validation of a DNA-Based Genomic Classifier for Prediction of Metastasis-Directed Radiotherapy Response in Oligometastatic Hormone Sensitive Prostate Cancer and Prognosis of High-Risk Localized Prostate Cancer

Prostate cancer that has spread to five or fewer parts of the body, known as oligometastatic prostate cancer, can often be treated with highly targeted radiation therapy to prevent metastatic progression. However, not all patients benefit uniformly, and tools that reliably predict who is most likely to respond to treatment are lacking. Similarly, patients with high-risk localized prostate cancer face varying risks of developing metastatic disease, and several different treatment approaches that combine radiation therapy and hormonal therapy exist. Making treatment decisions for these types of prostate cancer can be challenging and better tools based on patients’ specific tumor cell biology are needed. Our research focuses on a novel genomic biomarker called STRATOS-P that was discovered in a cohort of U.S. Veterans with metastatic prostate cancer. We will evaluate whether STRATOS-P can identify patients with oligometastatic prostate cancer who are most likely to benefit from metastasis-directed radiation therapy and determine which patients with high-risk localized disease are at greatest risk for developing metastatic disease. Our goal is to provide more personalized treatment recommendations without requiring additional testing. If successful, this work could help identify which patients with prostate cancer are most likely to benefit from radiation therapy.
